General Summary:

The Nurse Practitioner (NP) is an advanced practice RN and a clinical expert with a depth of knowledge, skills and education necessary for caring for a well-defined patient population.  The NP combines the roles of education, research and practice and is a consultant for a group of specialized patients and professionals.  The NP is responsible for managing patients and coordinating the services provided by multiple disciplines within and outside the health system.  The NP maintains a collaborative practice agreement with unit Medical Directors.  The NP excercises independent judgment in the assessment, diagnosis and initiation of selected protocols/procedures.


  1. Required Licenses and Certification:

    1. Maintains current State of Michigan Registered Nurse or Physician Assistant license. 

    2. Maintains Specialty Certification as a Nurse Practitioner or Physician Assistant from the State of Michigan.

    3. Maintains certification from a national Nurse Practitioner or Physician Assistant certifying agency such as the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners. 

    4. BLS certification or acquisition within 90 days of employment.

  2. Minimum Education:

    1. Master’s Degree in Nursing (M.S.N. or M.N.)

    2. Nurse Practitioner Certification or Physician Assistant Certification
  3. Skills/Abilities:
    1. Clinical expertise, leadership and organizational ability. 
    2. Able to communicate effectively and work collaboratively with others.
  4. Minimum Work Experience:
    1. At least two years Nurse Practitioner / Physician Assistant experience required. 
    2. Prefer minimum of two years advanced practice experience.

Essential Functions and Responsibilities:

  1. Assist the medical directors in managing the care of the patients.
  2. Functions in the role of advanced practitioner to coordinated comprehensive care deliver
  3. Writes order/prescriptions for standard formulary medication, laboratory, radiographic, studies and consultations, diagnostic evaluations, follow-up exams, laboratory tests, etc, in order to ensure continuity of care and coordination of services.
  4. Obtains histories and perform physical exams
  5. Makes daily rounds and be available for patient/family questions and answers.
  6. Acts as a liaison with physician/medical staff/specialists that consult on the unit.
  7. Communicate, collaborate and network with multiple disciplines to coordinate care for patient populations.
  8. Lead and monitor the development of individualized plan of care for each patient in collaboration with the medical director.
  9. Pursue self-directed learning experiences, continuing education, and maintain current standards as a nurse practitioner.
  10. Provide input regarding staffing needs from a clinical perspective.
  11. Discharge planning.
  12. Identify QI opportunities and assume responsibility for the overall quality of care on the unit.
  13. Work jointly with pharmacy and physician to review each resident’s medicine needs monthly.
  14. Performs all other related duties.
